Plot:
Kirsty Cotten is now grown up and married to Trevor Gooden. Her memory of the events that took place back at her parent's home and the mental institution have dimmed, but she is still traumatized. One fateful day, the two get into a fatal car crash, killing Kirsty. Now, Trevor finds himself in a strange world full of sexy women, greed and murder, making him believe he may be in hell. Trama:

Cuando el rompecabezas es encajado de nuevo, el devastador demonio Pinhead y su legión regresan para desatar el Infierno en la tierra y acabar con todo aquel que se oponga a ellos. Pero hay alguien dispuesto a hacerles frente, Kirsty Cotton (Ashley Laurence), la única persona que ha derrotado a Pinhead en el pasado y que puede salvar al mundo de esta terrible amenaza.
La película se estrenó en el 2002 y hasta el 2006 no fue doblada al español. Esta película fue dirigida por el maestro en efectos especiales Rick Bota.
